DetailsOnce you have completed all the learning activities for this course, you should be able to: - Understand the epidemiological aspect of T2DM and implement screening tools to aid in the diagnosis of DM. - Compile a comprehensive, patient centred lifestyle plan with patients that are living with DM. - Assess complex patients with added comorbidities to ensure control of glucose, blood pressure and lipid status. - Detect common but important complications of DM - Illustrate an understanding of the complexities surrounding the management of special populations that include but are not limited to pregnant patients, adolescents and children Description It is estimated that 6% of the South African population - 3.5 million people - suffer from diabetes and many more are pre- diabetic. Furthermore, diabetes is ranked as the second most common cause of death in the country. (STATS South Africa 2016) The majority of patients with diabetes seen in general practice and primary care will have type 2 diabetes. These patients often have additional co-morbidities which can be challenging to manage. However, given a certain level of knowledge and skill the healthcare practitioner will be able to optimise the care of these patients. A patient centred approach is essential to succeed in improving the outcomes of our patients with diabetes and their associated risk factors.
